 % of People with an SUD that have a success plan and warm hand-offs when re-entering society after incarceration.

This will require some programs to develop personalized success plans and make warm hand-offs, but once those programs are in place, it should be relatively easy to track the percent of people re-entering who have those in place. There will also need to be clear definition of what would count as a person who should be considered for this population (the denominator). Anyone who had misused opioids? Anyone who clearly experienced an opioid dependence and detoxification when they started their time of incarceration?

Possibly a care coordination platform.
 % of people with an OUD who are receiving MAT when released from incarceration.

While less comprehensive than a personalized, having at least some form of MAT would be a positive step for a person with an OUD re-entering after incarceration. Since the person should be sufficiently detoxed, long-acting naltrexone (Vivetrol or similar) could be an option. Research is needed.
